When someone regains their wits about them and realizes they're suddenly in some water with a PFD that they've been loosely wearing for the past 20 years, slipping-off around their neck, its fair to say that things aren't going as planned.
It's also not the best time to review proper PFD fitting or location and accessibility of the PPE kept in a SAR vest.
